Columns: provider name, contact first/last, type (Child Care Center / Group / Family Child Care Home / Drop-in / DOE / Exempt), regulatory agency TDHS/TDOE, regulatory status Licensed / DOE Approved / Exempt, capacity, QRIS score, hours, address, regulatory individual. No licence number (a claimed number is match None). Claimant printed as Provider Contact or in the provider name → verified; home with another contact → discrepancy; organisation otherwise → ambiguous_match. The TDHS Child Care Locator Map (onedhs.tn.gov ServiceNow, reCAPTCHA) renders the same roster. Teachers are TDOE (not covered).
